Patient Information

First Visit

Please assist us at the time of your initial visit to the office by providing the following information:

  • Your referral slip and x-rays from your referring dentist.
  • A list of medications you are currently taking.
  • If you have dental insurance, please bring any forms or insurance cards with you to the appointment.

Click here to download our New Patient Forms, which we encourage you to complete and bring with you to your first visit.

Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.

Please notify the office if you have a medical condition or concern prior to surgery (e.g. artificial heart valves or joints, heart murmurs requiring pre-medication, severe diabetes, or hypertension).

We will complete an in-depth medical and dental health history and a thorough examination to measure for bone loss, loose teeth, bite, oral cancer screening, TMJ, and other signs of periodontal disease.

Scheduling

We offer weekday and weekend hours, including early mornings and evenings to best accommodate your schedule.  We will schedule your appointment as promptly as possible.

Please allow approximately 1 hour for your initial visit. If you have pain or an emergency situation, every attempt will be made to see you that day.

We try to stay on schedule to minimize waiting at the time of your appointment. Referral of emergency patients can cause delays to the schedule, and your understanding and patience are very much appreciated.

Insurance, Payment, and Financing

Insurance
We accept all major dental insurance plans. If we do not currently participate in your plan, we will gladly consider adding your insurance provider.

We work closely with your general dentist to maximize your benefits and minimize your out-of-pocket expenses. We never let the insurance companies tell us what we can or cannot recommend to you as our patient based on your diagnosis. You can count on us to tell you exactly what you need in order to treat and control the diagnosed condition.

We spend the time necessary for you to understand what your insurance will (or will not) cover, financing options, as well as working around your schedule. Please remember though, many insurance companies will not release to us how much they actually cover on a procedure and will only do so to you. In those cases, we do our best to give you an accurate estimate but remember it is exactly that, an estimate. Rest assured that we are on your side and will always do our best to take care of you.

Payment and Financing Options
For your convenience, payments may be made with cash, personal check, Visa, Mastercard, or Discover.  We also offer attractive no interest financing options (if paid in full within 6 or 12 months) through CareCredit.  No interest promotional periods are available for all purchases, and interest will be charged to your account from the purchase date. If balances are not paid in full within the promotional period of 6 or 12 months, you will be responsible for all interest accrued from the purchase date.

Location and Directions
We are conveniently located in the Makefield Executive Suites at 301 Oxford Valley Road, Suite 506A, Yardley, PA, 19067, less than five minutes from I-95 and US-1.  When you enter the complex, make a left at the stop sign, continue straight for as long as the parking lot continues, then turn right and our office will be immediately on your right.
